Data Sharing Agreement

By accepting this document, the patient expressly authorizes the communication of necessary personal data to authorized hospitals, clinics, or surgical centers where Dr. Cristina Muñoz conducts surgical or medical procedures, pursuant to Article 6 of the GDPR and Spanish Organic Law 3/2018, for medical and administrative management.
